Nominate an outstanding third-year resident for the first ever
UAFP Family Medicine Resident Leadership Award
Help UAFP recognize the outstanding leadership and accomplishments of graduating residents from our four Utah family medicine residencies by nominating a deserving individual.
Nominations may be made by individuals who have knowledge of the nominee’s accomplishments including faculty, residency staff, fellow residents, and medical students. A nominee must be a resident member of UAFP and a current 3rd year resident at a family medicine residency in Utah. If you need to check a resident's membership status, please contact Barb Muñoz.
Nominees must demonstrate the following:
- Promotion of family medicine
- Leadership skills
- Serve as a role model to peers
Additional consideration will be given to those who excel in any the following:
- Teaching skills
- Professionalism
- Research skills
- Community service
How to Nominate:
Please answer the following questions in the form below OR provide a letter answering the following:
1) Is the nominee in good academic standing?
2) Give examples of how the resident has demonstrated promotion of family medicine, leadership skills, and acted as a role model to peers.
3) How has this nominee shown leadership in the residency and/or the community?
4) Any other examples you would like to include about why the nominee is deserving of this award?
Nominations are due no later than March 31, 2021. Winners will be selected by the UAFP Member Engagement Committee and announced in April 2021. If you have any additional questions, please contact Barb Munoz.
