Nominate a deserving colleague for the UAFP Family Medicine Physician of the Year!
The Family Physician of the Year Award recognizes a physician who provides their patients with compassionate, comprehensive and caring family medicine on a continuing basis.
This physician is directly and effectively involved in community affairs and activities that enhance the quality of their community.
They provide a credible role model, both professionally and personally, to their community, to other health professionals, and to residents and medical students.
This chosen physician effectively represents UAFP and the specialty of Family Medicine by presenting a good public image and exemplifying the family physician’s leadership role in improving the health of our state.
Please submit this completed nomination form below along with one to two (1-2) letter(s) of nomination. Letters may be written by an individual or committee/group. In the letter(s), please address the nominee’s:
- Current practice location
- Accomplishments and activities
- Exceptional qualities that position them as a leader among family physicians
- Impact to patients, community and/or the specialty of family medicine
Criteria on which nominees will be considered include the following:
- Patient Care
- Excellence in the Profession of Family Medicine
One or more of the following:
- Practice Leadership
- Community Service and Contributions
- Scholarly Activity
- Advocacy
- Experience and Career Accomplishments
- Commitment to the principles of equity, diversity, inclusion, and belonging
Nominations are accepted year-round and are good for three years from the date submitted. Nominations submitted within the previous three years by July 31 each year will be considered for that year's award. The winner will be recognized at the Annual Member Meeting held in the fall.
