Conference Registration


    Cancellation Policy:

    Registration cancellation requests that are made on or before January 15, 2021 will be honored for the full amount paid, minus the lesser of a $25 processing charge or 20% of the total paid. A cancellation fee of 50% of the registration paid will be assessed for cancellations occurring between January 15, 2021 and January 29, 2021. Cancellations on or after January 30, 2021 and conference no-shows are not eligible for a refund. This policy covers registration and ticketed events. Please allow up to four weeks for delivery of refund. The Utah Academy of Family Physicians is not responsible for fees or charges associated with payments, including but not limited to credit card, debit card or checking fees. Requests for cancellations and refunds must be directed to Barb Muñoz via email at munozb@utahafp.org phone at 801-736-0722.

    In order to provide an ABFM-endorsed KSA group study session, we will utilize Zoom so that attendees can interact with speakers, take part in question and answer polls, and chat with fellow KSA participants. The ABFM also requires that the program administrators verify that participants are paying attention throughout the duration of the program. If you register for this session, please plan on setting aside the full four hours to fully participate.
