Payment Information:

Exhibitors are required to pay an initial deposit of 50%. Final payment is due no later than January 11, 2020. If full payment is not received by January 11, 2020, registration is forfeited.

Cancellation:

Payment is fully refundable if exhibitor cancels by December 13, 2020 minus a $25 processing fee. Payment is 50% refundable if exhibitor cancels by January 10, 2020. No refunds will be issued after January 10, 2020.

I have read and understand the conditions of this contract. By checking below I am indicating my organization/program’s agreement to abide by the UAFP’s rules and regulations for CME & Ski exhibitors as the same may be amended from time to time. I accept responsibility for informing all of our representatives of these conditions and for ensuring that they will abide by them also. I further understand the violation, cancellation policies and have reviewed the complete list of rules and regulations.
