Conference Registration


    Optional Family Member Breakfast

    Breakfast for family members can be purchased for $120 per person for the conference. Breakfast served Monday, Tuesday, and Wednesday morning. The $120 charge covers one family member for all three breakfasts. You do not need to purchase a breakfast for children 5 and under. Please indicate number of additional breakfasts you would like to purchase.

    Cancellation Policy:

    Registration cancellation requests that are made on or before December 13, 2024 at 11:59pm MST will be honored for the full amount paid. A cancellation fee of 50% of the registration paid will be assessed for cancellations occurring between December 14, 2024 at 12:01am MST and January 10, 2025. Cancellations on or after January 11, 2025 and conference no-shows are not eligible for a refund. This policy covers registration and ticketed events. Please allow up to four weeks for delivery of refund. The Utah Academy of Family Physicians is not responsible for fees or charges associated with payments, including but not limited to credit card, debit card or checking fees. Requests for cancellations and refunds must be directed to Barb Muñoz via email at [email protected] phone at 801-736-0722.

    Please read: The Utah Academy of Family Physicians (UAFP) is not responsible for problems beyond our control, including but not limited acts of God; inclement weather, fires, flood or other emergency conditions. UAFP reserves the right to cancel an event due to low enrollment or other circumstances which would make the event non-viable. If UAFP cancels this event, registrants will be offered a full refund. Registration fees cannot be refunded in the event that weather conditions prevent or hinder attendance. The Utah Academy of Family Physicians will make every effort to accommodate attendance by registrants in spite of any extenuating circumstances. However, in registering for this event, registrants acknowledge and understand the inherent risks associated with this venue.
