Payment Information:

Exhibitors can pay in full or pay an initial deposit of 50%. Final payment is due no later than January 10, 2025. If full payment is not received by January 10, 2025, registration is forfeited.

Cancellation:

Payment is fully refundable if exhibitor cancels by December 13, 2024 minus a $25 processing fee. Payment is 50% refundable if exhibitor cancels by January 10, 2025. No refunds will be issued after January 10, 2025.

If event is canceled due to including but not limited to strikes; labor disputes; acts, regulations or orders of governmental authorities; civil disorder; disasters; acts of war; acts of God; fires; flood or other emergency conditions; any delay in urgent and essential repairs of the Hotel; or any other extraordinary occurrence outside the control of the Utah Academy of Family Physicians making it impracticable, illegal or impossible to provide or use the Hotel facilities or creating a significant risk to participant health or safety; non-performance is excused without further liability of any nature and any deposit (if any) shall be returned to the sponsor/exhibitor. Deposit(s) shall be returned ninety (90) days upon date of cancelation due to force majeure. Sponsors/exhibitors will be notified within seven business days of event cancellation.

I have read and understand the conditions of this contract. By checking below I am indicating my organization/program’s agreement to abide by the UAFP’s rules and regulations for CME & Ski exhibitors as the same may be amended from time to time. I accept responsibility for informing all of our representatives of these conditions and for ensuring that they will abide by them also. I further understand the violation, cancellation policies and have reviewed the complete list of rules and regulations.
